Equally important, though less visible, are the support services team who maintain our appointment schedules, support our computer system and process our documents, referrals and recalls.

The team is composed of: Community Nurses who assess, plan and deliver care, including wound care, continence assessment and care and end-of-life care. Community Matrons who have extended clinical knowledge and skills and work with people in their homes to help them manage their health problems and prevent unnecessary admission to hospital. Community Nurses for Older People CNOPs who work with older people to promote good health and spot early deterioration of their condition.

They will work with people in their home for an extended period of time. Healthcare Assistants who support the work of the Community Nurses. Patients who are being cared for by the Community Nursing team can contact them on: Monday - Friday — on Out of Hours - The Health Visitors provide supervision and support for children from birth to five years old, monitoring growth, development and the well-being of the child within the family.

They can be contacted directly by parents or carers on Children over the age of five are seen by the School Nursing Service which can be accessed through the child's school. The midwifery team provide all mothers with care through pregnancy, labour and for two weeks post-natally.
